We strongly encourage you to contact your insurance provider to verify coverage and benefits with DK Physicians PLLC, our contractor that provides physician services. Any co-pays, deductibles, or non-covered services must be paid by the patient and will be charged to the card on file. Any cancellations or changes must be paid in full by the patient.
