In-network and out-of-network coverage is the patient’s responsibility. We strongly encourage you to contact your insurance provider to verify coverage and benefits with DK Physicians PLLC, our contractor that provides physician services. Any co-pays, deductibles, or non-covered services must be paid by the patient and will be charged to the card on file. Any cancellations or changes must be paid in full by the patient.
